Sleep appointments for initial therapy set-up's and mask/supply appointments will require confirmation to make sure that your benefits and authorizations from your insurance are obtained before your appointment. All co-pay's and deductibles are due at time of service. If you have questions about your anticipated financial responsibility please call our office 866-363-3678. To book by phone please call 757-251-2718 M-F 9am-5pm. All co-pay's are due at time of service. There is a $25 fee for missed appointments.
